This agreement is entered into between the Lake County Bar Association (hereinafter, "LCBA") and the above-referenced member (hereinafter, "Member").

1.       LCBA maintains a website on the Internet at:  www.lcba-ohio.org. 

2.       As part of the website, LCBA maintains a Roster of its membership at no charge to its members.  LCBA also maintains a listing of areas of concentration, or "Categories".

3.       LCBA offers listings in the categories below to its members for an annual fee.

4.       A Member wishing to subscribe to any of the following Categories shall indicate by a checkmark in the box to the left of each Category.  The name, address, e-mail address, telephone number, and fax number will be linked to his/her name, and this shall be known as the "Basic Listing".  If Member wishes to link his/her own website to the Basic Listing, there will be a one-time charge of $10.00.

Wherefore, the parties agree as follows:

A.      The minimum listing charge is two categories, or $50.00, per member.  For each category checked below, Member shall pay $25.00. The listing shall run for a period of one year from July 1^st of each year through June 30^th of the following year, and shall be prorated, if necessary.  This contract will renew annually at the then scheduled rate and payment shall be made to LCBA.  Should Member fail to make any payment due under this agreement, LCBA shall have no further obligation to host or maintain the Category listing or web page of Member.

B.      Member agrees to pay the sum of  $_______________(# of Categories selected X $25.00) for the Categories selected below.  Changes made to the listing shall be made by LCBA at no charge to Member once during each year of subscription.  Thereafter, charges for changes shall be at the rate of $5.00 at the time of the change.  No changes shall be made until payment is received.

 C.  Member may add a photo to their webpage for a fee of $25.00 with an annual renewal fee of $5.00. 
       Submit the photo digitally by email or a hard copy by mail to LCBA.
